Piccolo = small handle; Grande = big handle. “Testina Gentile” is the closed-comb head and it’s available with either handle. They don’t have a specific designation for the open comb model afaik.
Neither handle is Large. I would describe the Piccolo as Teeny (like a ball-end Tech) and the Grande as Medium (similar to a typical Merkur / Edwin Jagger / Muhle handle).
Those are both pretty decent razors with their own quirks. Both can be quite comfortable and give good close shaves once you master them. Both are durable as heck.
I would probably get one of the Grande Gentile instead of Piccolo Gentile razors if you go the Fatip route though; I prefer the larger handle, personally.
It should have two edges (hence the name “double edge”) - the soap gets balanced on one of the edges
